Among patients with acute ischemic stroke and hyperglycemia, treatment with intensive vs standard glucose control for up to 72 hours did not result in a significant difference in favorable functional outcome at 90 days. These findings do not support using intensive glucose control in this setting (JAMA)

Incretin-Based Antihyperglycemic Agents for the Management of Acute Ischemic Stroke in Patients with Diabetes Mellitus: A Review
Several preclinical studies consistently showed that GLP-1 receptor agonists reduce infarct volume and improve the functional outcome after ischemic stroke. These beneficial effects appear to be mediated by anti-inflammatory, antioxidant, and anti-apoptotic actions of these agents (Diabetes Therapy)
Prevalence of diabetes and its effects on stroke outcomes: A meta‐analysis and literature review
Approximately one‐third of all stroke patients have diabetes. Uniform methods to screen for diabetes after stroke are required to identify individuals with diabetes to design interventions aimed at reducing poor outcomes in this high‐risk population (Journal of Diabetes Investigation)
Pioglitazone after Ischemic Stroke or Transient Ischemic Attack
In this trial involving patients without diabetes who had insulin resistance along with a recent history of ischemic stroke or TIA, the risk of stroke or myocardial infarction was lower among patients who received pioglitazone than among those who received placebo (NEJM)
Evaluation of low-dose aspirin for primary prevention of ischemic stroke among patients with diabetes: a retrospective cohort study
In this study of patients with diabetes, the use of low-dose aspirin showed an increased risk of hospitalization for ischemic stroke. These results suggest that low-dose aspirin use for the primary prevention of ischemic stroke should be reconsidered in patients with diabetes (Diabetology & Metabolic Syndrome)
